Carolina Massage Institute Student Enrollment Agreement

  • Student is enrolling in the Massage Therapy Diploma Program of Study, including 505.5 clock hours of instruction and practicum. Program begins on January 8, 2018, and ends on June 21, 2018. Program lasts 24 weeks and is 505.5 clock hours. Expected graduation date is June 21, 2018.

    Student is responsible for all financial obligations, including $9,500 tuition ($6500 if paid before Oct 1, $8500 if paid before Nov 30, or according to the following payment plan: total $10,500, with the schedule as follows: $500 deposit upon acceptance, with 5 payments of $2000 due two weeks before the program begins (Dec 25), Feb 1, March 1, Apr 1, and May 1), and an expected $300 for a massage table, $200 for books, and $100 for miscellaneous supplies.

    A $50 application fee is due at time of application. Upon acceptance, this fee is applicable toward tuition.

    Upon completion of this agreement, a $500 non-refundable (except in the instance of program cancellation by CMI) deposit is required to reserve the student's place in the program.

    Tuition is due in full on or before 7 days before the start of the program.

    Should a student choose to cancel enrollment within 72 hours of signing a student enrollment agreement, all monies paid less a $100 processing fee will be refunded.

    Should a student choose to withdraw from the program before the program start date, all monies paid less a $100 processing fee and $500 non-refundable deposit will be refunded.

    Should a student choose to withdraw from the program after the program start date, no refund will be issued. Should there be extenuating circumstances that require a leave of absence or for the student to withdraw from the program for good cause, tuition paid will be refunded at the discretion of the Director.

    Should a program be cancelled by CMI, all monies paid will be refunded in full.

    Student has a right to cancel enrollment at any time. Should a student choose to withdraw from the program for any reason, this notice shall be submitted in writing to the Director.

    CMI accepts cash, check, money order, and VISA, Mastercard, Discover, and American Express. Upon acceptance to the program, student is encouraged to speak with the Director to determine a plan for payment. This may include installments to the school, personal bank provided loans, and/or CMI work study scholarships. No interest will be charged for installment payments, provided that the tuition is paid in full on or before the start date of the program, or according to the payment plan detailed above. If tuition is not paid in full prior to the start date of the program or according to the student's payment plan, student will not be enrolled in the program, and no refund will be issued for any partial payment submitted.

    Student acknowledges and understands that completion of the program does not guarantee licensing or job placement.

    Student must complete all coursework to the satisfaction of the requirements set forth in the School Catalog. Failure to complete this work may result in dismissal from the school. Additionally, students my be dismissed from the school if it becomes evident that the student lacks good moral character, poses a disciplinary problem within the school, and/or does not adhere to the standards of conduct outlined in the School Catalog. Students dismissed from the program for cause will not be eligible for reentry and will not be issued a refund for any tuition paid.

    Anyone wishing to practice massage and/or bodywork in the state of NC must apply for and receive a license from the NCBMBT. Licensure requirements include successful completion of at least 500 hours of training at a BMBT approved school's massage therapy program (like CMI) and successful completion of the MBLEx licensing exam. Additionally, the North Carolina Board of Massage and Bodywork Therapy may deny a license to practice massage and bodywork therapy if an applicant has a criminal record or there is other evidence that indicates the applicant lacks good moral character.

    Before completion of this Enrollment Agreement, students will be issued the CMI School Catalog, which fully outlines all school courses and policies.
